In the event you cannot make health care decisions for yourself, an Advanced Care Plan can ensure your wishes, based on your values and beliefs, are followed. Health decisions include who you want to make decisions for you, health care treatments you agree or disagree with, acceptance or refusal of life support and life prolonging interventions.
When these decisions are in place, it helps the medical team know how to proceed with your care. This can ease distress caused by the need to make decisions without the time to consider what quality of life means to you. An Advanced Care Plan relieves the stress and anxiety, and sometimes guilt, on family and friends when they have to make decisions on your behalf.
